Your Visit

Please arrive 15 minutes before your scheduled appointment to allow time to part and register. Eye exams often require more time than other doctors' appointments, particularly if your doctor finds a problem that requires additional testing.

We make every effort to accommodate necessary testing at the time of your visit, alleviating the need for your return on another day.

These online forms are provided for the convenience of new or establish patients who have scheduled appointments.

If you are a new patient, or haven't been seen at Eye Associates for Fort Myers for two years or more; please complete each of our online forms.
